ONLINE APPLICATION FOR ADMISSION TO STD. V

IMPORTANT INSTRUCTIONS

• Applications for admission to Std. V (2023-2024) shall be submitted online, as per the schedule detailed in the Annexure I (Schedule for Admission to Std. V)
• Applicants should have a valid e-mail ID and a valid cell phone number.

Documents

Before filling the online application form, parents are requested to keep scanned images of the following documents in.pdf format (size should be less than 2 mb) for purpose of uploading:

1. Statement of Marks of Std. IV
2. ID card or Report card of brother/sister, if any, presently studying in Bhatikar Model High School
3. School Leaving Certificate or passing certificate of the parent, if parent is ex-student of the school
4. Caste Certificate, if applicable(Issued by Govt. of Goa)
5. Proof of Residence (Address specified in the online application should be the same as mentioned in the document submitted as Proof of Residence) (Any one of the following)
6. Documents:
   1. Aadhaar Card
   2. Domicile / Residence Certificate issued by Competent Authority
   3. Election Identity Card
   4. Telephone Bill
   5. Property Tax
   6. Ration Card
